Large histiocytes strongly stained by S100 immunostain clearly showing intracytoplasmic lymphocytes (emperipolesis), which is characteristic of Rosai-Dorfman disease. S100 negatively outlines the lymphocytes within the cytoplasm. Often Rosai Dorfman lesions show significant fibrosis with only rare cells showing emperipolesis and hence careful examination and high index of suspicion is critical.
