Instant Haemolysis by Complement activation due to High Titre Cold Agglutinins

1. Bed-side sample drawing resulted in compliment mediated immediate haemolysis in a plain vial [Red top].

2. Critical analyses while drawing the sample revealed that the haemolysis was relatively rapid, nearly maximum occurring within one minute at a temperature range of 22-25⁰C.

3. A comparative inclusion of di-potassium ethylene diamine tetra acetic acid [EDTA K2; Purple top] vial for drawing sample from the same patient was completely clear, notably so due to calcium already chelated, with subsequent avoidance of compliment activation in the vial.

4. In addition on immune-haematological workup mono-specific direct coombs test was positive for “anti-C3d” denoting a cold agglutinin of a wide thermal amplitude [20 to 37⁰ C and titre greater than 1000 causing rapid haemolysis in the plain vial.
