Pseudo-thrombocytopenia in a Healthy Whole Blood Donor

1. Pseudo-thrombocytopenia (falsely low platelet count) was suspected in a 21 year old healthy whole blood male donor without any obvious signs of petechiae or ecchymoses.

2. The hematological values obtained from his ethylenediaminetetraacetic acid (EDTA) sample (prior to donation) using a routinely calibrated automated cell counter (Sysmex K21, Japan) were: Hemoglobin = 16.8 g/dL; hematocrit = 51.3%; platelet count = 57,000/µL; all red cell indices within normal limits.

3. To confirm the same, we repeated his platelet count using citrated blood sample which gave normal valve (153,000/µL) while, his peripheral blood smear revealed aggregates of giant sized platelets.

4. Platelet aggregation is a common in-vitro phenomenon usually seen when blood is collected in an EDTA vial.
